NEW HOOF
A part completed copy of the Home Oxygen Order Form (HOOF) with the important bits for CH'ers pre filled in. This is a new form to be used with immediate effect - the previous version will not be accepted from 1st November 2009. We have, at long last, managed to get cluster headache listed as a category (number 20) rather than "other". Progress, indeed!
Note: Please read the third page.
This part completed form is ONLY to be used for CH sufferers and other related Trigeminal Autonomic Cephalalgia sufferers.
