A part completed copy of the Home Oxygen Order Form (HOOF) with the important bits for CH'ers pre filled in. Note: Please read the third page.
This part completed form is ONLY to be used for CH sufferers and other related Trigeminal Autonomic Cephalalgia sufferers.
A transcript of the presentation given by Professor Peter Goadsby at this years conference is available for download. Please go to the download section.
